Complete the form below and submit to us to sign up to volunteer for the Kentucky State Fair - Lions Vision Screening Booth.  You do not need to be a Lions Club member to volunteer and no experience is necessary.  Training is provided on site and all volunteers receive FREE admission and parking at the KY State Fair!  Join us!
